WAIVER OF LIABILITY AND RELEASE AGREEMENT

This Waiver of Liability and Release Agreement (the “Agreement”) is entered into by and between the undersigned individual (the “Client”) and [Spa Name] (the “Spa”), as of the date signed below.

  1. ACKNOWLEDGMENT OF RISKS

I, the undersigned, acknowledge and understand that spa treatments, including but not limited to massages, facials, body treatments, hydrotherapy, sauna, and other wellness services, involve certain risks. These risks may include, but are not limited to, allergic reactions, skin irritations, muscle soreness, dizziness, burns, or other unforeseen physical effects. I voluntarily assume all risks associated with my participation in these services.

  1. MEDICAL CONDITIONS AND CONSULTATION

I acknowledge that I have disclosed all relevant medical conditions, allergies, injuries, and sensitivities to the Spa before receiving any services. I understand that the Spa does not provide medical advice and that its staff are not healthcare professionals. I have consulted with my physician regarding any conditions that may affect my ability to safely receive treatments.

  1. WAIVER AND RELEASE

I hereby release, waive, discharge, and hold harmless the Spa, its owners, employees, agents, and representatives from any and all claims, liabilities, demands, damages, costs, and expenses, including but not limited to bodily injury, illness, allergic reactions, or any other harm that may arise from my participation in spa treatments, except in cases of gross negligence or willful misconduct by the Spa.

  1. COVID-19 AND COMMUNICABLE DISEASES

I acknowledge the risk of exposure to communicable diseases, including but not limited to COVID-19. I voluntarily assume this risk and agree that the Spa is not liable for any illness that may result from my visit.

  1. PERSONAL RESPONSIBILITY

I agree to follow all Spa policies, instructions, and recommendations provided by staff. I understand that failure to adhere to these guidelines may result in my removal from the premises without a refund.

  1. GOVERNING LAW

This Agreement shall be governed by and interpreted in accordance with the laws of the state in which the Spa is located.

  1. SEVERABILITY

If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
